Specializing in weight management, the services  offered are geared towards weight loss/gain or weight maintenance for adults, adolescents, elderly, and those with pre-existing diagnoses such as: diabetes, hypertension, and other cardiovascular/metabolic conditions. Together, we’ll create a custom meal plan, discuss physical activity recommendations, and address/modify your current barriers and behaviors.  If weight loss isn’t your primary focus,  that’s okay. Our team is equipped to address your concerns and help you meet whatever nutritional goals you may have.

Consultation

A complimentary phone call or face-to-face discussion that allows us to become better acquainted with each other. We can discuss goals, services offered, and answer any questions we may have for each other, to see if working together is a good fit.

Initial Visit

Dietary assessment: all relevant information is gathered via discussion and questionnaires that goes into designing your individualized plan. Nutritional needs calculation: calorie, macro- and micronutrient targets are determined. Meal Planning: with your input, weekly meal plans are designed according to your food preferences, nutritional targets, # of meals you choose daily, schedule, and more. Establish weekly/ biweekly/ monthly goals: together, we set goals to work towards until your next visit.

Follow-up Visits

Monitor and evaluate goals: progress is measured in efforts to make sure we’re on track for accomplishing your overall goal(s). Discussion: review successes and struggles, nutrition education, behavior modification, and establish new goals and interventions such as: altering calorie and nutrient intake, adjustment in exercise/physical activity, incorporating new foods, etc.

Out-of-pocket Packages

6 week: one initial and five subsequent follow-up visits

12 week: one initial and eleven subsequent follow-up visits

18 week: one initial and seventeen subsequent follow-up visits

Health Insurance

At Upper Bucks Weight Management, we accept the healthcare plans listed below. Please confirm your individual plan’s coverage for medical nutrition therapy or we can check for you. Also, many plans offer preventative care benefits where there are no costs to their members (no co-pays or deductibles).  For plans out-of-network, clients pay upfront and are provided with a super-bill that they can then submit for partial or full reimbursement.
